Vaman (Therapeutic Vomition ) -
Introduction
• Vaman = Therapeutic vomiting or induced
vomiting
• Differences ,in disease process like vomiting
,due to Hyperacidity or vomiting with loose
motion in Gastro & Vaman as Treatment
• Disease - vomiting is Natural protective
mechanism of the body -- Ayurveda uses this
Nature’s principle as a tool of Treatment

Vaman – Introduction
• Impure Doshas from the body are removed
from the mouth by induced vomiting
• The procedure is known as vamana
• Vaman is the cleansing process in which
mainly kapha dosha ,& secondarily vitiated
Pitta Dosha is removed out from the body
• This treatment modality requires the
Proficiency or skill of vaidya

Vaman – Indications
• It is mainly indicated in kapha dominated
diseases – like ---
• Gastrointestinal conditions like Anorexia,
Indigestion, Hyperacidity originated from
Indigestion & Not due to eating spicy food or
due to NSAIDS or due to alchohol
•
• Recurrent stomatitis ( Pitta Problem)

• Respiratory conditions like --
• Cough or Bronchial Asthma with lot of
expectoration ,& not in dry cough or dry
Asthama
• Chronic sinusitis
• Bronchiectasis
• Repeated tonsillitis

• Cardiovascular conditions like –
• Congestive heart disease, Hypertension,
Anemia
• Neurological conditions like ---
• Epilepsy & Dementia

• Hormonal problems like amenorrhea,
scanty menses, PCOD & infertility due to
tubal block , Hypothyroidism, Obesity
• Lymph adenitis
• Skin problems like - Recurrent pustules
on skin, Psoriasis, Urticaria
• Recurrent middle ear infections

Happy in Long run --
• Vaman process can be described as
Nisukhayatvam Sukhaya cha !
• This means process itself is not so comfortable
to follow but after process benefits are
rewarding
So makes the patient happy in long run .
• For example in Bronchial Asthama patients
Intensity & frequency of attack is definitely
reduced after Vaman taken in Spring(Vasant )

Caution for Vaman
• Needs good physical & mental strength
of the patient
• So caution should be taken while giving
Vaman for Vata or Pitta dominant
Biotype or Vata ,Pitta Diseases ( Rather
these can be Contra Indications for
Vaman)

Vaman - Contra Indications
• Delicate conditions or Delicate people like --
• Pregnancy,
• Children, Old age,
• Very thin or very fat person,
• Patients of cardiac problems like Angina or MI,
• Weakness or general debility due to chronic
or degenerative diseases like Last stage of
Cancer or AIDS or Tuberculosis

Vaman –Contra-Indications
• Bleeding disorders especially from upward
part of the body like Haemetemesis
,Haemoptysis ,Epistaxis
• Actually Ayurveda says ‘Ptrati margam Dosha
haranam ,is the Treatment principle for
Raktapitta Vyadhi
• Removing Doshas from the opposite direction
is the principle

• For example if there is Female patient heavy
Menstruation Vaman can be tried
• If there is Epistaxis tendency in Summer
try for Virechan ( Therapeutic Purgation)

Vaman –Contra -Indications
• Ascites ( Nityam Enam virechayet is the principle for
Udar)
• Poor digestive power
• Ayurveda says not only Vaman ,but any
Panchakarma should not be performed in Sama or
condition with Ama ( Toxin like substance ,due to
Indigestion at Jatharagni or Dhawagni level)

Deepan –Pachan –Before Vaman
• So most of the times ,before planning
Panchakarma ----
• Deepan & Pachan medicines are given
• Like Agnitundi Vati or Amapachak vati or
Shankha vati
• Musta or Nagarmotha is Best for
Amapachan

• Any disease or serious problem in chest region
or ENT region like --
• Pleurisy, Throat infection, Aphonia
• CA –Larynx ,CA –Lung


• As vaman is strenuous process – So
contraindicated in -----
• Persons doing strenuous Physical work
or exercise
• People having too much stress
• Diseases of vata doshas mostly
degenerative conditions like wasting

• Helminthiasis
• Patients who have taken Anuvasan
or Niruha basti.

Vamak Dravya- Emetic Drugs
• Medicines that are used to induce vomiting
are called as vamak dravyas. Most commonly
used formula is given here with
• Madan phala (Randia dumetorum)-3 Gm +
Yashtimadhu (Glycyrrhiza glabra)-2 Gm
+Vacha (Achorus calamos)-1 Gm + Honey- 20
ml + Rock salt- 2 to 3 gm

Vaman Drugs –
Pharmacodynamics & Pharmacokinetics
• Ushma (Hotness) -- With this quality, vaman
dravyas liquify the doshas
• Tikshna (Sharpness) -- With this quality, sticky
doshas are separated from strotasas
• Sukshma (Minuteness) -- Because of this
quality, medicines can reach up to minute
channels (strotasas) in the body and perform
their function.

Vaman Drugs –
• Vyavayi- Easily distributing --
• Because of this quality, medicines don’t
need to go through the first pass
metabolism
• Its action starts right from its entrance in
the body
• Hence vaman drugs act rapidly

Vaman Drugs –
• Vikasi --
• This quality helps to spread the medicine and
destruct binding between the particles of
impure doshas
• These medicines have agni (fire) and vayu
(air) mahabhutas predominance

Vaman Drugs –
• But not all the plants or substances with
these qualities can cause induced
vomiting
• Induction of vomiting is the characteristic
( Prabhav = Special Functional Capacity)
of these plants or substances

Vamnopaga dravyas - Supportive emetics
• Medicines that help the action of Vamak
Dravyas to induce vomiting
• Some of them are ----
• Yashtimadhu (glycerrhiza glabra) Decoction
• Sugar cane juice
• Milk
• Salt water

How the process of Vaman takes place?
• Vaman drugs absorbed easily and spread all
over the body.
• They bring the vitiated Doshas from the
tissues to the stomach i.e. from shakha to
koshtha.
• Due to proper implementation of pre
therapeutic procedures like Snehana,
Swedana , the Vaman procedure occurs
smoothly without any complications

Pre treatment of Vaman
• Firstly examine the patient carefully whether
he is fit for vamana or not
• Patient should be mentally prepared for the
treatment
• Then better Informed written consent should
be taken.
• Although procedure is safe ,if done it in a
scientific manner & by experienced vaidya .

• But still considering very rare complications
like Cardio Pulmonary arrest or Haemoptysis
or Haemetamesis or Dehydration ---
• It is advisable to take written consent & better
this procedure should be done in hospital set
up ,where emergency if at all occurs ,can be
managed easily

• If the patient is fit for vaman then start
pretreatment i.e. snehana (internal and
external Oleation) and Swedana (fomentation)
as stated earlier
• Examination of digestive power can be done
by giving the testing dose of oily substances to
the patient
• This testing dose is termed as ‘Rhasiyasi
matra’ of Snehana

• Time taken by the patient to digest this dose
decides the proper dose. This testing dose is
generally of 50 ml
• So mathematical Equation will be as follows
• If Patient is takes 3 hours for Digestion of 50
Grams ,then How much will be quantity to
Digest in 12 hours ? Answer will be 200 ml .
This will be the last day dose to get Samyak
Snigdha Lakshan

• The quantity of sneha to be taken is
increased gradually for 3- 5-7 days in Mrudu,
Madhyam ,Krur Koshtha respectively
• On the next day of Samyak or adequate
Snehapan i.e completion of internal oleation,
next day external oleation and sudation is
given. On the next day Vaman is given with
prior external oleation and sudation. It means
for vaman there is only one day break.

Symptoms of Samyak Snehan
• 1) Sneha Vidwesha ( Nausea of Sneha
Substance)
• 2) Adhastat Sheha Darshan ( Stool becomes
oily)
• 3) Twak Snigdhata ( Skin becomes oily or
shining)

• After the appearance of the symptoms of
proper Oleation ( Samyak Sneha) , internal
Oleation should be stopped
• Give one day break and on the next day one
should go for the process of vaman

• During these two days massage
( Abhyanga or Full body oil massage) and
fomentation ( Nadi Sweda or Steam box)
should be continued
• During the period of pretreatment
patient should be examined regularly

Main procedure of Vaman
• The dinner on the day before of Vaman ,------
• Kapha vardhak or Kapha increasing diet is
advised like ---
• Udid or black gram vada with curd or Rice &
curd ,Rice with milk, Banana, Sadha Rawa
dosa without curry etc
• It increases kapha, kleda and facilitates their
removal from the body through vamana

• Also Triphala decoction enema in a dose of
200 to 300 ml is given in the evening before
the Vaman.
• At the day of the procedure, patient should
come to the clinic by drinking 1 glass of milk
at home & may bring mixture of curd,
jaggery,1 Banana and salt. Which should be
taken in clinic
• This will help to make Kapha dosha Uttkat .

• Then the medicines are given, those
induce vomiting ( Vamak drug )
• Generally madanphal pippali (seed of
madanphala) is used for induction of
vamana along with vamanopaga dravyas

• The quantity of drug depends on the strength of the
patient.
• After administration of vamak drug (Madan phala
(Randia dumetorum)-3 Gm + Yashtimadhu
(Glycyrrhiza glabra)-2 Gm +Vacha (Achorus calamos)
-1 Gm + Honey- 20 ml + Rock salt- 2 to 3 gm )
• Then the patient should be given External massage
specially to chest, back, neck etc and swedana ( Nadi
Sweda)

Vitiated Doshas from Shakha to Koshtha
• Then first body hair will stand & forehead
sweating will appear ,which indicates ,vitiated
doshas are starting separation from body
tissues
• Then there will heaviness in stomach &
abdominal discomfort ,which indicates
Doshas have reached into stomach from
peripheral tissues

Vitiated Doshas from Shakha to Koshtha
• Lastly there will be too much
Nausea -----
• Indicating ,Vitiated Doshas ( Kapha)
are ready to be expelled out

• Then immediately start giving decoction of
Liquorice ( Jashthamadha) ,2 to 3 glasses at a
time
• It is administered as much amount the patient
can take
• Totally for Nice Vaman to take place ,4 to 5
liters is required
• After that, vomiting starts spontaneously

• Vomitus is collected and measured
• Liquids are given repeatedly to the
patient up to the complete removal
of kapha dosha from the body.


4 Examinations for Vaman
• 4 Types of Examinations are done –
• i) Antiki  ---  Pitta should be eliminated last
• ii) Vegiki  --  usually 6 to 7 big bouts of Vaman
are adequate
• iii) Maniki  -- Measurment examination is not
so important
• iv) Laingiki  -- Observe symptoms of Samyak
vaman

• After the complete removal of kapha
dosha, there is removal of small
amount of pitta dosha from the body


• At this last point of completion ,patient has bitter
taste with some times little burning , in the throat &
mouth
• Vomitus is yellow colored. This stage suggests that
the process of vamana is successfully completed
• After that the liquids should not be given to the
patient
• During the process of vamana, pulse & blood
pressure of the patient should be monitored now &
then, to avoid complications

Cooperation of Patient is Essential
• Always remember that this
procedure cannot be conducted
without co-operation of patient
• so his or her physical and mental
power should be in a good status at
the every moment of vaman

Symptoms of proper vaman karma
( Samyak Yog)
• Removal of Primarily vitiated kapha ,followed
by pitta
• After removal of vitiated doshas, episodes of
vomiting stops automatically
• Feeling of lightness in chest ,abdomen and
head + Freshness of the body
• After vaman karma, patient may feel
weakness for couple of days
•

Symptoms of incomplete vaman
karma ( Ayog)
• No expulsion of impure doshas, only
expulsion of vaman dravyas
• Feeling of tightness in the chest & abdomen
• Heaviness in the body
• Uricaria rash, itching, fever
• In incomplete vamana, impure doshas are
retained in the body. They can create many
other diseases.

Symptoms of excess vaman karma
( Atiyog)
• Considering this complication ,written
consent before Vaman is advisable
• Vomiting with fresh blood or blood
stained
• Symptoms of dehydration- Dryness of the
mouth, excess thirst , giddiness,
tachycardia etc. Even hypovolumic shock

After treatment of Vaman
• Patient’s mouth ,face ,chest ,abdominal
part should be cleaned by towel ,soaked
in warm water
• Person should take rest for some time
• Stay in a room devoid of heavy breeze

After Vaman –Give Pot
• Patient should be provided some pan
• Because after vaman when patient is
shifted to indoor ward ,patient may
get 1 or 2 bouts of vomiting or there
may be some excessive salivation &
patient may want to spit

After Vaman – Loose motions possible
• Even some patients may get 1 or 2
times loose motions
• Patient should be informed about
that

After Vaman –Give Dhupan
• Dhumapana- Generally it is done with a
specially prepared stick
• Dhuma varti or Aturvedic Cigar like
Nirdosha
• Some times we can put Bajara flour on
hot plate & fumes coming from it can be
inhaled

After Vaman –Give Dhupan
• In vamana, impure dosha is situated in
stomach is excreted through mouth
• Some impurities may remain in throat,
mouth and nose
• For removing of of this remnant of
kapha dosha, dhumpana is indicated

Variechanic Dhumapan
• Mostly Vairechanic dhumpana is done with
Manahshila, haratal (purified arsenic
compounds), dhattur patra (Dhattura
stramonium), Aguru (Aquilaria agallocha) etc.
• If above drugs are not available, Vacha
powder or Pippali powder is also used
• Especially in asthmatic patients pippali is used
for dhoomapan

What is Sansarjana krama ?
• Follow the Sansarjana krama (Post therapeutic
regimen)
• Immediately after Vaman karma, digestive
power of the patient becomes poor
• The digestive power is restored by following
specific Dietary plan after completion of
shodhana treatment
• It is termed as sansarjana krama.

Sansarjana krama
• In this process, firstly light, liquid diet is
advised to the patient
• From Liquid diet ,patient is brought to normal
diet within a span of 5 to 8 days
• For example first 2 days liquid diet like luke
warm water,Laja manda, Kharjur month,
Vegetable soups, Fruit juice, Medicated herbal
Teas etc --Then next 2 days soft & light diet
like rice, Khichadi

Sansarjana krama
• Next 2 days ----
• Jwar or Bajara Bhakari +
• Green leafy vegetables
• Then normal diet but --
• Less in amount & avoiding spicy ,oily
food

Don’ts after Vaman
• After vaman, patient become weak i.e.
convalescent phase and hence avoid following
things, which will vitiated Tridosha
• Excess talking, Excess Physical Exertion,
suppression of natural urges, long vehicle
journey, sleeping during day time, not
sleeping during night, avoidance of sansarjana
krama

Step by step for Rasayan -
• If someone wants to perform the whole
panchakarmas --
• Then after vaman karma, do the
sansarjan krama and then go for virechan
karma
• One have to do snehan, swedan again as
a pretreatment for Virechan


Shaman after Shodhan
• If no other panchakarmas are to be
performed then ----
• After sansarjana krama go for medicinal
therapy which may be ---
• Rasayan (for rejuvenation) or may be
shaman (therapeutic).

Vaman In Clinical Practice

• 1) In Bronchial Asthama –
• Vaman can reduce the frequency and severity
of the attacks.
• Reduces the requirement of the
bronchodilator medicines & Inhalers.
• Need to be given once a year ( Preferably in
Vasant rutu or spring season ,in the month of
March & April) for 3-5 years

• 2) Vitiligo & Psoriasis-
• Results are good if vaman is given in the
primary phase of the disease
• But need to be given on & off
• For Pre –Snehan – Use Panch Taikta
Ghruta

• 3) Acne Vulgaris-
• This is consequence of Vitiated Pitta dosha
attacks on Rasa , Rakta and needs the
purification.
• Vaman is beneficial in this condition along
with Blood letting with local application of
Leeches

• 4) Vaman is indicated in Menorrhagia i.e. excess
bleeding at and between the menses- (Artava is
linked with Rasa Dhatu .Raja is Updhatu of Rasa
dhatu .According to Ayurveda Excess Menstrual
bleeding can be compared with Adhoga Raktapitta
.Raktapitta Management Principle is Pratimargam
cha haranam .This explains that Downward bleeding
can be controlled by Vaman process & upward
bleeding can be managed by Virechan)

• 5)Vaman is indicated in Pyoderma and
secretary or weeping Eczema.
•
• 6) Hyper acidity – In such cases vaman and
then virechan shows excellent results.
Complaints of repeated emesis due to
hyperacidity are found to be reduced ( Hyper
acidity arising from Indigestion )


• 7) PCOD –
• As Raja is the upadhatu of Rasa dhatu ,
Amenorrhea in PCOD is considered as
avarodha ( obstruction) in Rasa dhatu.
• Vaman in this cases helps to remove the
obstruction.

• 8) Productive Cough in children –
• Though childrens are contraindicated for
vaman , in a productive cough (pathology of
vitiated kapha) vaman can be given by licking
two pinch of vacha powder with Honey
• Child vomits twice or thrice and then he get
fresh and active

• 9) Urticaria --- Vaman reduces allergic
tendency & urticaria can be cured
permanently
• 10) Allergic Rhinitis --- Vaman reduces
allergic tendency
• 11) Vaman can reduces Hyper
cholesterol level in the blood

Animals Know Vaman
• Cats & Dogs – when get abdominal Pain due to
indigestion –
• Just eat some grass –Get vomited by
themselves
• And Get back to normal again
• Can we human learn anything from this ?

Nausea may be indication of Vaman
• In some patient –Due to Indigestion ,Nausea
starts --- when go to Doctor –Anti Emetic
drugs are given – Aam or Toxins are
suppressed – Recurrence may occur
frequently
• But in the same patient when lot of vitiated
Kapha or Pitta are showing tendency to come
out – Indicated by Nausea –These patients
feels better after Vomiting

Psychological problem & Vaman
• Vaman ,not only cleans Kapha – but also
lessen Raja ( Irritability of Mind ) & Tama (
Inertia or Depression of mind )
• Case – of Acute Anxiety Depression to a
Teacher – Pre Vaman Snehan by Brahmi
Ghruta – followed by Vaman –Patient slept
well & improved further by Shirodhara etc

Diabetes Mellitus & Vaman
• Ayurveda recommends frequent Vaman –
Nearly once in each month
• In IDDM = Insulin Dependent Diabetes
Mellitus + Over weight + Sedentary Life Style
•
• Vaman may be stimulating – Beta cells of
Pancreas or Activating receptors of Insulin

Skin problems & Vaman
• Can avoid use of External or Internal Steroids
• Psoriasis ,Wet Eczema, Vitiligo, Urticaria
repeated boils or Pustules
• Stop Incompatible food like Milk shake or milk
Banana

Dyslipidemia & Vaman
• Hypercholesterol –more than 200 mg % --
• Chances of Atherosclerosis –IHD
• Cholesterol comes down after Vaman
Treatment + Add Shaman Chikitsa with
Nagarmotha ( Aama pachak) ,Triphala ,Vidang
,Aarogyavardhini Tab ,Owadi Guggul Tab
• Even this helps to reduce coronary blockages

TB & Vaman
• Vaman can be given even in TB pts but under
strict supervision of Expert & Experienced
Vaidya
• Pre snehan should be done with Vasa Gruta &
Vaman can be given with only Madanphal with
Honey paste & then Milk to it’s maximum
quantity like 2 to 3 liters
• Post Vaman only warm water can be given in
spite of Salt water

Caution
• Caution should be taken to avoid Vaman
Complications
• Do BT ,CT,PT & ask history of any Bleeding
disorders ,Oesophageal varices ,Peptic ulcer
etc
