背景:急性冠動脈症候群の病態生理

虚血症状は必ずしも明確でない
背景:症例
• 倦怠感にて来院した87歳女性
• 日中に農作業を行い、夕方から下肢の脱力感を自覚して来院
• 高血圧にて近医内科で降圧剤処方あり
• 生命兆候:呼吸状態・循環動態は安定、意識清明
• 理学所見:下肢脱力を除き特記所見なし
• 胸部レントゲン:特記所見なし
• 血液検査:高CPK血症と腎機能障害あり
• 熱中症の診断で補液目的に入院とした
目的及び方法
• 目的:
• 典型症状を主訴としない急性冠症候群の診療実態の把握
• 見逃しを最小にするマネージメント方法の提案
• 方法:
• 調査:診療録による後方視的検討
• 期間:2013年4月1日〜2016年3月31日
• 対象:以下を満たす患者
• 聖隷三方原病院高度救命救急センターを受診、ないし、救急車来院
• 主訴が「胸痛」「呼吸困難」以外であったもの
• 初期診断が急性冠症候群
• 解析:Microsoft Excel 2016
結果1:胸痛以外が主訴のACSの頻度
• 総患者数:22,326例
• 急性冠症候群(初期診断):539例
• 胸痛、呼吸苦以外が主訴:26例 (4.82%)
• 問診困難例(CPA・意識障害):8例
• 経過中に胸痛、呼吸苦を自覚:9例
胸痛・呼吸苦を呈さない症例は
急性冠症候群症例の1.67%

ERにおける無痛性心筋梗塞の疫学的検討

  2. 開示すべきCOIはありません。
  3. 本邦における死因を提示いたしますが、ご存知の通り、心疾患は2位となっており、その中でも虚血性心疾患は全体の1/3程度を占める重要な疾患群と言えます。
  4. 急性冠動脈症候群は冠動脈の不安定プラークにより生じる心筋虚血による疾患と考えられます。 この診断においては、典型的な症状と心電図変化、血清学的変化によって成され、冠動脈造影によって確定されます。 しかしながら、高齢者や女性、糖尿病患者などにおいては、典型的な症状を有さないことが知られています。
  5. ここに症例をお示しします。 高血圧などで錦衣通院中の女性が、夏場に屋外で作業をした後の倦怠感で受診されました。 理学所見では下肢脱力を除き特記所見なく、採血検査では筋原性酵素の上昇を認めており、熱中症の診断で入院加療の方針となりました。 しかし、その後の入院前スクリーニングで取られた心電図で下壁梗塞が示唆され、緊急冠動脈造影となり、#2にPCIの後、CCU入室となりました。
  6. このような症例の見逃しを防ぐ戦略を立案するべく、聖隷三方原病院高度救命救急センターを救急車来院ないし独歩来院後入院となった症例を3年度分解析しました。
  7. 観察期間中、22,326例がエントリーされ、そのうち、539例がACSと診断されました。 その内、約5%の26例が胸痛/呼吸苦を主訴としておりませんでしたが、CPAや意識障害を8例含み、さらなる問診で経過中に典型症状を呈していた症例が9例ありました。 即ち、純粋な意味での典型症状を有さない症例は1.67%の9例と非常に稀であることが示されました。
  8. 胸痛/呼吸苦を主訴としない26例の主訴はこのようになっており、比較的心疾患を疑い易い症例が多く含まれました。 危険因子としては年齢が圧倒的であり、冠危険因子を1項目以上満たす症例が9割以上でした。
  9. 検査特性としては、ECG/L/Dで診断可能な例が多く、危険因子と心電図で26例中25例が診断可能でありました。 また心電図による部位診断としては下壁梗塞が最も多く含まれ、前壁梗塞がそれに続きました。 26例中1例のみがCAGで責任病変を認めず、たこつぼ心筋症と診断されました。
  10. 狭義の無痛性心筋梗塞は極めて少ないものの、致死的な疾患であり、見逃しを最小限にする戦略が必要であると考えられました。 胸痛以外を主訴とする急性冠症候群の診断において、危険因子と12誘導心電図はいずれも感度が高く、これらを組み合わせることでスクリーニング可能であると見込まれました。 さらに、これらが陰性でもACSが否定できない病歴の患者ではTnIの採血及びObservationが必要と考えました。
